ABSTRACT

A support and comprehensive care for a ovarian cancer survivor is meant by providing them information pertaining to their disease , treatment and end results . Its aimed at educating them for self care and motivating attendants and training them for providing continuous, physical, emotional, financial, psychological support, Still, regular follow-ups can also help understand their concerns and address them immediately before they impact. Questionnaires have been the best way so far, to evaluate caregivers’ satisfaction levels, lack of information, and attention. With this approach, the common hindrances can be cleared before impacting the caregiver’s mental and physical health.

ABSTRACT

Background and aim: The introduction of rotavirus vaccine (RVV) in the universal immunization program of India is a big feat as it became the first nation in the World Health Organization (WHO) Southeast Asia region to do so. The involvement of huge numbers of frontline workers in introducing new vaccines in India and the underlined deficits in skills and knowledge require efficient capacity building programs. In view of this, limited research is available on the effectiveness of capacity-building interventions for healthcare workers. There is a dearth of studies from India measuring the “on-spot” impact of immunization trainings on healthcare workers. This study aims to assess the effectiveness of training in RVV introduction in enhancing the knowledge of the participants. Methods: The study was conducted among the participants attending two training workshops for the introduction of RVV: a state workshop in Pune and a regional workshop in Guwahati. The participants who attended the workshops and participated in both the pre- and post-test were included in the study. Real-time data was collected via Google forms pre- and post-training sessions. Results: In both workshops, a comparison of pre- and post-test scores of all questions taken together showed a significant increase in the knowledge level of the participants (p < 0.05). In Guwahati, the knowledge of the participants regarding doses of RVV, inadequate dosing, vaccine vial monitor (VVM), open vial policy, operationalization of RVV and monetary incentive increased significantly. In Pune, the knowledge of the participants regarding doses of RVV, bundling approach, schedule and dose, storage temperature for RVV, VVM, open vial policy, vaccine delivery and operationalization of RVV increased significantly after the training. Conclusion: A pre-planned and well-designed knowledge assessment tool can be used to understand the impact of training workshops in enhancing the knowledge and practical skills of the participants prior to the introduction of a new vaccine.

ABSTRACT

Nizatidine is an anti-secretogogue and a gastroprotective drug with a half-life of 1-2 h and is well absorbed in the stomach. This study aimed to optimize the process and develop floating microparticles of nizatidine that are based on low methoxyl pectin. Oil-in-oil dispersion method and Taguchi orthogonal array design were employed, and the prolonged residence time of the microparticles in the stomach was demonstrated. The constraints for independent variables, viz. A-polymer, B-internal solvent volume, C-surfactant, D-stirring rate and E-stirring time were set to generate the experimental runs. Particle size, percentage yield, micromeritic properties, entrapment efficiency, in vitro buoyancy and in vitro release were characterized. Surface morphology, zeta potential, in vitro release kinetics and in vivo floating performance of the optimized formulation was examined. The microparticles were free-flowing, irregular in shape and had a mean particle size distribution of 73-187 µ. Low methoxyl pectin played a predominant role in achieving buoyancy and optimum gastric retention for the modified release of the drug, suggesting Korsmeyer-Peppas model as the possible release mechanism. In vivo radiographic study in rabbits revealed that the drug was retained in the stomach for a period of 6 h. These results indicate that nizatidine floating microparticulate system provides modified drug release for the effective treatment of gastric ulcer

ABSTRACT

Chemotherapy Induced Peripheral Neuropathy (CIPN) can be precipitated by many modern chemotherapeutic agents. CIPN affects the patient’s quality of life not only physically, but also functionally, psychosocially, spiritually and affects the family as well. If severe acute syndrome develops during chemotherapy, doses of drugs should be reduced or even stopped. CIPN is a neurological side effect occurring during chemotherapy treatment in cancer patient depending on many factors such as age, dose strength, complete total drug dose received, period of therapy, use of more than two groups of neurotoxic agents, concomitant neuropathies (for example, diabetic neuropathy), genetic vulnerability, and alcoholism. Pathophysiology of CIPN is not clearly understood. The pathological processes by which chemotherapy drugs harm the nervous system structures cause CIPN. It depends on several factors or causes which include microtubule disruption, oxidative stress and mitochondrial break, changes in ion channel activity, myelin sheath and DNA damage, and neuro inflammation. Clinical manifestations vary from patient to patient. It is classified into three types such as sensory, motor and autonomic. Presently there are no standard guidelines for the assessment of CIPN. It is considered both objective evidence of neurological dysfunction and assessment of symptoms reported by the patient. Nurses can play a very important role in the early identification of CIPN in cancer patients which can help to plan and in modification of treatment. This will help patients to enjoy a better quality of life. This review aims to give an update regarding CIPN and nursing aspect.

ABSTRACT

The NOVEL COVID 19 pandemic’s blowout from the PRC with its spread across the Globe has left a heavy impact on the mental health of the people. Measures and strategies evolved to contain the virus are public awareness, personal protection and hygiene, social distancing, testing kits etc. The related mental health issues are to be addressed. We wanted to evaluate the impact of COVID19 pandemic on the mental health of the people across the Globe. METHODSMental health related to COVID 19 outbreak, for the last five months from December 30, 2019: the published articles; abstracts; editorials; copyrights, have been considered and reviewed, from Sources: PubMed; Medline; and Cochrane database library. RESULTSThe physical and social distancing has resulted as social isolation. The self-quarantine has led many to anxiety, depression, mood changes, insecurity, low self-esteem and post-traumatic stress disorder. Many are may not be able adapt to present situation. CONCLUSIONSMore Interventional studies are required to attend mental health during / after this Pandemic. Encouraging productive activities at home can prevent the mental disorders. Proper structuring cognizance by the institutions and online services is required to educate and rehabilitate the victims across the country and globe.

Type II diabetes mellitus (T2DM) is closely associated with Obstructive sleep apnea (OSA) and obesity. Type 2 diabetes and OSA may be pathophysiologically independent conditions although the joint association with obesity or visceral adiposity. There is a consistent relationship between obesity and OSA, which has been reported in 60-90% of OSA patients. The prevalence of obesity increases with a parallel increase in the prevalence of OSA. Continuous positive airway pressure (CPAP) therapy is an effective choice of treatment for OSA, an overnight test, or titration some patients may reduce apnea events by minimizes airway collapse by CPAP. Several studies showed that the effect of drug treatment with 3 months of C-PAP in patients with type 2 diabetes. In the present study, we include 300 patients in different groups, out of the 100 patients undergoing treatment of CPAP therapy minimum for three months. Blood sugar, HbA1c, and lipid profile were measured and an overnight sleep study was done. The obtained data shows the significant effect of therapy on physiological and biochemical parameters. AHI and BMI were highly significant in group II and Group III when compared to group I. FBS, HbA1C, and Lipid profile parameters also gave significance results (p-value <0.001) in group II and group III when compared with healthy subjects (group I).

ABSTRACT

This review summarizes the state of the current literature relating to the associations of lung disease and adipokines (proteins produced by adipose tissue) in humans. The mechanistic basis for these associations in humans is not established, although a possible role for adipokines has been invoked. Leptin, a pro-inflammatory adipokine, and adiponectin, an anti-inflammatory adipokine, are causally associated with asthma in mice. Although human studies are currently inconclusive, high-serum leptin and low-serum adiponectin concentrations predict asthma, independent of obesity, in select population groups, such as premenopausal women in the United States. In contradistinction, low-serum leptin and high-serum adiponectin concentrations are associated with stable COPD, although these associations are likely confounded by fat mass. Interestingly, leptin may promote systemic and airway inflammation in stable COPD patients. On the other hand, COPD may upregulate systemic and lung adiponectin expression. The precise mechanism and significance of the associations between these adipokines and lung disease at the current stage are confusing and frankly paradoxical in places. It is now known that adipose tissue is not an inert organ simply for energy storage, but regulates systemic inflammation via a variety of secreted proteins (called adipokines). While the associations of obesity and adipokines with cardiovascular, endocrine, and rheumatological diseases are well described, the respiratory effects of obesity and adipokines are less well known. This review will focus on the effect of obesity and adipokines on asthma and chronic obstructive pulmonary disease (COPD) in humans. This area of research needs additional study that may open up novel therapeutic strategies for these lung diseases.

Gingivitis is a reversible and non-destructive form of periodontal disease. Oxidative stress contributes in the pathogenesis of periodontal diseases5. The oxidative stress has been implicated as one of the important contributory etiologic factors in many of the oral inflammatory pathologies including gingivitis. This research analyzed the "Total antioxidant capacity" (TAC) of biological fluids including saliva. The present cross-sectional study was conducted to evaluate the total antioxidant capacity (TAC) of saliva in children with/ without gingivitis and its relation with Age and Gender. For measuring the TAC of saliva: Cayman's Antioxidant Assay Kit was used and Gingival Index Measured through The Gingival Index (Löe and Silness, 1963). The results were analyzed using descriptive statistics and making comparisons between cases and control by using SPSS software version 20. In this result, mean TAC of saliva in case children group was found lower 0.203 ± 0.053 compared to control children group was higher 0.236 ± 0.048. While, in male and female children of aged 3-5 years were found antioxidant activity (TAC) lower in compared to control groups. But among males aged 6-13 years it was found that the mean antioxidant capacity of saliva in case group was 0.259 ± 0.040 while in control group it was 0.295 ± 0.026. The TAC of saliva in males was found high compared to female. A weak negative correlation was found between the TAC and gingival index. In conclusion TAC decreases in children with gingivitis compared to healthy children. The gingivitis was more observed in female leading to lower TAC value

Diabetic nephropathy (DN) is a chronic complication of both type 1 and type 2 diabetes. However, there is still inadequate understanding of the exact mechanism related to progressive diabetic renal disease. The GLUT-1 XbaI gene polymorphism in the glucose transporter has been suggested in the development of DN. However, its association with T2DM and DN is controversial and has not been established in different ethnic populations. To enhance the understanding of GLUT-1 XbaI gene polymorphism in the context of T2DM and DN. We investigated the possible genetic association of GLUT-1 XbaI polymorphism with T2DM and DN in North Indian population. 100 T2DM patients and 100 patients of DN with 100 healthy controls were included in the study. GLUT-1 XbaI polymorphism was determined by PCR (polymerase chain reaction) and RFLP (restriction fragment length polymorphism). The obtained data showed no significant association between GLUT-1 XbaI gene polymorphism with T2DM and DN leading us to conclude that GLUT-1 XbaI gene polymorphism may not have major effects on T2DM and DN in North Indian population.

Iron deficiency anemia (IDA) is one of the most common nutritional disorders worldwide, affecting people of all ages in developed and developing countries. causes dimunition of various antioxidant enzymes like superoxide dismutase, catalase, glutathione peroxidase, glutathione reductase. The objective of the study was to detect impact of iron supplementation in anaemic & non- anaemic pregnant subjects on oxidative stress. Patients are divided into two groups, Control groups (60 non-anemic pregnant women) & Study groups (60 anemic pregnant women). The blood sample (5 ml) was collected from different groups of subjects. In controls groups, a fall in catalase, superoxide dismutase, glutathione peroxidase and glutathione reductase were seen while lipid peroxidase was found to have increased significantly after iron therapy. In study group, and increment in all markers except lipid peroxidase was seen and the level of lipid peroxidase was decreased following iron supplementation which was statistically significant (p<0.001). it may be concluded that iron deficiency anaemia is associated with free radical generation and peroxidation of vital body molecules which implies increased risk for pregnant women.

Hypothyroidism is a syndrome resulting from thyroid hormone deficiency or rarely inefficacy. It is a common endocrinological problem affecting especially women and the elderly. Thyroid hormones play an important role in synthesis, metabolism and mobilization of lipids. One of the most important symptoms of hypothyroidism is weight gain or inability to lose weight. The rapid increase in the prevalence of obesity in the past 20 to 30 years emphasizes the important role of lifestyle and environmental factors, because genetic changes could not have occurred so rapidly. Obesity is a complicated process that depends on signals of satiety and hunger, genetics, endocrine abnormalities, and other factors. Our study purposes to investigate the relationship between BMI and thyroid function in patients of subclinical hypothyroidism. Objective of this study is to correlate thyroid stimulating hormones (TSH) and body mass index (BMI) in male and female patients with subclinical hypothyroidism. This cross sectional study included 42 Subclinical hypothyroid patients, both males and females between 20-45 years of age and without a history of alcohol or tobacco consumption, history of any cardiovascular disorders or diabetes mellitus and any drug history like corticosteroids, beta blockers. and TSH test estimated by VITROS 5600 integrated analyzer and correlated with body mass index. The results showed a positive Correlation analysis in subclinical Hypothyroid males between BMI and TSH, (r= 0.47), which was found to be statistically significant (p=0.02*) in case of subclinical Hypothyroid females the results indicated positive correlation between BMI and TSH (r= 0.58) which was found to be statistically significant (p=0.005*). A strong positive correlation between Body Mass Index (BMI) and Thyroid stimulating hormones (TSH).

Hemoglobin is the protein molecule present in red blood cells that carries O from the lungs to the body tissues and returns CO from the 2 2 tissues back to the lungs. The red blood cells have the ability to concentrate hemoglobin in the cell fluid up to about 34 grams in each 100 ml of cells. Anemia refers to a reduced oxygen carrying capacity of blood either due to reduced red blood cell count or decreased hemoglobin concentration. Anemia in women apart from many other causes is possibly due to the result of systemic inflammation which is probably because of the smoke of biomass fuels. To study correlation between hemoglobin levels and use of biomass fuel (wood) for cooking by females in rural areas of Lucknow district. The examination was done on the 44 females between 18 to 55 years of age and cooking food using biomass fuels for at least 5 years. Women who were pregnant, having history of bleeding disorders like hemophilia and Thalassaemia, history of tobacco intake, history of medications like steroids and history of hemorrhoids were excluded from the study. Examination of the blood sample for hemoglobin estimation was collected in the rural health training center (RHTC) of Era's Lucknow medical college and Hospital and hemoglobin estimation was done in the hospital lab services (HLS) Era's Lucknow medical college and hospital, which use fully analyzing method SYSMEX XS 8001for hemoglobin estimation . The correlation between biomass fuel (wood) smoke and hemoglobin levels was strongly positive and statistically highly significant (p= 0.004). The decrease in hemoglobin levels due to exposure to indoor air pollution resulting from the burning of biomass fuels in smoky fires for cooking have important implications for diagnosis of anemia.

Muscles are the only tissues in the body that have the ability to contract and hence move the other parts of the body. The strength of any given muscle, in terms of force exerted on the skeleton depends upon length, shortening speed, cross-sectional area, sarcomere length, myosin isoforms and neural activation of motor units. Fatigue is a reversible physiological state in which there is an objective reduction in the performance or it's absence resulting from continuous or prolonged activity. The degree, duration and type of work are the important factors that affect the performance and onset of fatigue. To correlate body mass index (BMI) and muscle fatigability in young healthy adults. This cross sectional study included 100 apparently healthy young adults, both males and females between 18-24 years of age and without a history of alcohol or tobacco consumption, history of any cardiovascular disorders or diabetes mellitus and any drug history like corticosteroids, beta blockers. and underwent fatigue test using Mosso'sErgograph and the work done (Kgm) was calculated and correlated with body mass index. The results showed a strong positive correlation between Body Mass Index and work done(r=1) which was found to be statistically significant (p<0.05). Moreover, The results showed a very strong positive correlation(r=1) between different categories of Body Mass Index and the amount of work done by the exercising muscle, but the results were statistically non significant (p<0.05) in all the sub categories of body mass index. A strong positive correlation between Body Mass Index and amount of work done by the exercising muscle suggests that the level of fatigueness in an individual is related to his/her nutritional status and physical fitness.

Obesity has become a matter of quality to health care administrators. The busy lifestyle of people made them prefer fast food instead of taking healthy food. But the people are not aware that fast food habit converts to the disease like obesity, type 2 diabetes mellitus (T2DM), dyslipoproteinemia etc. This case control study had been carried out in department of Physiology in collaboration of Department of Biochemistry and Medicine, Era's Lucknow Medical College & Hospital, Lucknow to explore status blood sugar fasting (BSF), tumor necrosis factor-α (TNF- α), Insulin by standard spectrophotopmetric kit methods, blood pressure (BP) as well as anthropometric measurements with the help of suitable instruments and equipments in Control group, Obese group and Obese with type 2 diabetic group. Values of all above parameters were found increased in obese group with respect to control group and values of all these parameters were found increase in obese with type 2 Diabetes mellitus group with respect to obese group. Thus it is clear that obesity is risk factor for T2DM, Dyslipoproteinemia and coronary artery disease (CAD).

This study had designed, to explore prevalence of obesity and overweight, on two criteria, Indian and World Health Organization (W.H.O.), amongst students and staff of Era's Lucknow Medical College & Hospital, Lucknow and Era Institute of Health Sciences and Research, Lucknow. As well as, assessment of association of oxidative stress with obesity. MBBS, Nursing and DMLT students as well as Hospital Laboratory Services (HLS) staff had included in this study. Study had carried out in Department of Biochemistry with collaboration of Department of Physiology and HLS. Results of this study had showed that, prevalence of obesity and overweight were high in staff with respect to students. The levels of reduced glutathione (GSH) were decreased in obese with respect to normal; on the other hand Malondialdehyde (MDA) levels were increased in obese with respect to normal.

Introduction: The confusion around the diagnosis of ‘medically unexplained symptoms’ has lead to a paradigm shift in criteria for diagnosis of somatization disorder. Aims: 1. To compare the socio-demographic variables in patients of somatization disorder 2. To compare the levels of depressive and anxiety scores of patients of somatization disorder along with the severity of disorder. Material and Methods: Somatization patients visiting the psychiatry outdoor of TMMC & RC, were randomly selected and diagnosed as per DSM-IV TR. After obtaining informed consent and applying exclusion criteria, demographic and clinical details were obtained on a self designed Performa. The HAM-A scale and MADRS scale were applied to calculate anxiety and depression scores. Results: The prevalence of somatization disorder was 2.35% in men and 6.7% in women. Females were significantly higher in number. Headache was the chief complaint. The anxiety scores and MADRS scores were highest in patients complaining of chest pain. The HAM-A and MADRS scores increased significantly as number of complaints increased. Female patients and patients belonging to rural background had significantly higher number of complaints. Illiterate patients had a significantly higher duration of illness. Conclusion: Somatization disorder comprises unique group of patients with high co-morbidities and longer duration of illness. It is imperative to identify and clarify severity of this subgroup as treatment decisions need to be modified accordingly.

The present paper deals with the field observations recorded on the traditional indigenous therapeutic applications of the plants Butea monosperma (Lam.) seeds used by the inhabitants of the Sagar district, M.P. (India). Even today a number of plants of the local flora are used for curing various ailments and diseases. The information is given in a tabular form as scientific names of plants in alphabetic order followed by family, part used, effective against, and target animals. Information on local/vernacular names of plants, uses, parts used names of ailments and modes of usage are given in detail. Information on traditional uses and commercial uses as well as biological activities of the related species is included on the basis of the existing relevant literature so as to present a comprehensive account. In this review we compiled available literatures from libraries, scientific journals and online database query’s on plants and remedies used in traditional medicinal systems for such diseases.
